Support Coordination helps you understand your NDIS plan and get the most out of it. Your Support Coordinator works with you to find the right providers, get your supports working together and build your confidence to manage your plan over time.

You spend less time chasing providers and more time on your goals. Step by step, you learn how your plan works, so you can make more of your own choices.

Recovery Coaching is for people living with psychosocial disability related to a mental health condition. Your Recovery Coach walks alongside you to build hope and resilience, set recovery goals and put the right supports in place, at your pace and on your terms.
People with psychosocial disability who have Psychosocial Recovery Coaching funded in their NDIS plan. If you are not sure, we can check your plan with you.
Coaching is practical and hopeful. You choose the goals, and your coach helps you take steps towards them. Our approach is recovery-oriented, trauma-informed and strengths-based. Sessions can be in person, by phone or by video.
Capacity Building: Support Coordination. Psychosocial Recovery Coaching is usually funded in this budget when it is included in your plan. Our prices follow the NDIS Pricing Arrangements and Price Limits.
Daily Living Support helps you with everyday tasks at home and helps you build the skills to do more for yourself. We follow your routines and respect your culture, your privacy and the way you like things done.
People who would like support with personal care, meals, medication or daily routines, and who have daily living support funded in their NDIS plan.
You can stay safe and comfortable at home and build the confidence to do more on your own. Our workers do things with you, not just for you, so your skills can grow over time.
Core Supports: Assistance with Daily Life. Help to build life skills may be funded from your Capacity Building budget, depending on your plan. We explain our prices in your service agreement before we start.
Household Tasks support helps keep your home clean, safe and comfortable. We help with the jobs your disability makes hard to do, so you have more time and energy for the things you enjoy.
People who find some household jobs hard to do because of their disability, and who have household tasks funded in their NDIS plan.
A clean, tidy home is safer and easier to live in. You do the jobs you want to do, and we help with the rest.
Core Supports: Assistance with Daily Life. Household tasks are funded when your plan includes them, usually because your disability makes these jobs hard to do yourself. Our prices follow the NDIS Pricing Arrangements and Price Limits.
Community Participation support helps you get out, try new things and feel part of your community. We support you to go where you want to go, meet people, and build friendships and confidence.
People who want support to take part in social, community and recreation activities, and who have this support funded in their NDIS plan.
Being part of your community can lift your mood, grow your skills and open new doors. We go at your pace, and you make the choices.
Core Supports: Assistance with Social, Economic and Community Participation. Some skill building may be funded from Capacity Building: Increased Social and Community Participation. The NDIS does not usually pay for everyday costs like tickets, entry fees or meals.
Travel and Transport support helps you get where you need to go. We can support you to get to appointments, work, study and activities, and help you learn to travel on your own.
People who find it hard to get around or use public transport because of their disability, and who have transport or travel support in their NDIS plan.
Getting around opens up your whole week. With travel training, you can learn to travel more on your own, when you feel ready.
Core Supports: Transport. Transport linked to your activities may also be claimed with Core Supports: Assistance with Social, Economic and Community Participation. Travel training may be funded from your Capacity Building budget. We explain all transport costs in your service agreement before we start.
Accommodation and Living support helps you find the right place to live and the support to live well there. We can help you arrange short stays and respite, Supported Independent Living (SIL) and support with your tenancy and home.
People who want a short break away from home, would like support to live more independently, or need help to find and keep a home. What we can arrange depends on your NDIS plan.
The right home and the right support make everyday life easier. A short stay can give you a break, a change of scene and a chance to try new things, and it can give your family or carers a rest too.
Core Supports: Assistance with Daily Life funds STA, respite and SIL when they are in your plan. Help to find and keep a home may be funded from Capacity Building: Improved Living Arrangements. We can help you understand what your plan includes.
Home Modifications make your home safer and easier to live in. From grab rails and ramps to bathroom and kitchen changes, we coordinate the whole process: assessment, approval, building work and follow-up.
People whose disability makes it hard to move around or use parts of their home safely, and who have (or need an assessment for) Home Modifications funding.
Well-designed changes can reduce falls, make daily tasks easier and help you stay in the home and community you know.
Capital Supports: Home Modifications. Most changes need an OT assessment first. Larger or more complex changes also need NDIA approval before work starts. We guide you through each step.
Build strength, fitness and confidence in a way that works for your body and your goals. Sessions are adapted to your abilities and can happen at home, in the community or at a local gym.
People with health and wellbeing goals, such as moving more easily, building strength, managing weight, or using exercise to support mood and sleep.
Regular exercise can make everyday tasks easier and help you feel good. Where it helps, we work with your physiotherapist or exercise physiologist.
Depending on your plan and goals. Personal training may be funded from Capacity Building: Improved Health and Wellbeing, and how your plan is managed also makes a difference. We can help you check before you start.

Under the NDIS Code of Conduct you have the right to be treated with respect, to make your own decisions, to privacy, to safe and competent support, and to speak up without fear. We take these seriously. See our Safeguarding commitment.

Families and carers know the person best. With the participant's consent, we keep you informed and involved.
We invite family and carers to planning meetings and reviews, share progress updates, and agree with the participant how much you'd like to be involved. Their choices and privacy always come first.
Caring is demanding. We can help you find respite options in a participant's plan, connect you with carer support services and aim to give you one clear point of contact.
You'll have a direct contact for the person coordinating supports, regular check-ins, and we aim to call you promptly if anything changes. We can communicate in your preferred language with an interpreter.
We work with Local Area Coordinators, Support Coordinators, hospitals, schools, GPs, allied health and community organisations across Western Sydney and South Western Sydney.
You'll receive confirmation of intake and progress updates at agreed intervals, and we aim to tell you promptly about any concerns, with the participant's consent. We welcome your feedback on how we can work better together.

We're always looking for caring, reliable people, especially bilingual workers who reflect the communities we serve.
Respect, honesty and consistency. We match workers to participants thoughtfully, keep rosters stable and back you with a supportive office team.
Induction, the NDIS Worker Orientation Module, ongoing training and pathways from support work into coordination and coaching roles.
Regular supervision, debriefs after difficult shifts, clear policies and a team you can call. You'll need an NDIS Worker Screening Check, a Working with Children Check (for relevant roles), first aid and a driver's licence.

The National Disability Insurance Scheme (NDIS) is the Australian Government scheme that funds supports for eligible people with permanent and significant disability. Your funding is set out in your NDIS plan, and you choose the providers you want to work with. You can call the NDIS on 1800 800 110.

How soon supports can start depends on the service, your plan and finding the right worker for you.
Yes. A family member, carer, Support Coordinator, Local Area Coordinator (LAC), health professional or other provider can refer you, with your consent. They can use our referral page. You can also refer yourself.
Yes. We can explain how the NDIS works, how to apply and who to talk to. You can call the NDIS on 1800 800 110 to ask about applying. Once you have a plan, we can talk with you about the supports that fit.

We offer nine services: Support Coordination, Psychosocial Recovery Coaching, Daily Living Support, Household Tasks, Community Participation, Travel and Transport, Accommodation and Living, Home Modifications and Personal Training. See all our services.
Both help you use your plan well. Support Coordination focuses on finding and coordinating providers across all your supports. Psychosocial Recovery Coaching is for people with psychosocial disability, and it combines coordination with recovery-focused coaching. Your plan will say which one is funded.
Daily Living Support can include personal care, meal preparation, daily routines and building life skills. It can also include medication support, which means prompts and assistance in line with your medication plan. Read more about Daily Living Support.
Yes. Our Household Tasks service can include cleaning, laundry, yard maintenance, gardening, lawn mowing and bin days. These are funded when your plan includes them, usually because your disability makes these jobs hard to do yourself. Read more about Household Tasks.
Yes. Community Participation support can help with shopping, appointments, outings, sport, recreation and group activities. Travel and Transport support can help you get there, and travel training can help you learn to use public transport. We explain any transport costs in your service agreement.
Short-Term Accommodation (STA) is a short stay away from home, with support. It can give you a break and a chance to try new things, and it can give your family or carers a rest. This is often called respite. We can help you find and arrange STA when it is included in your plan. Read more about Accommodation and Living.
SIL is ongoing help with daily tasks in the home where you live, so you can be as independent as possible. Many people who get SIL share a home with others. The NDIA decides on SIL funding after looking at your home and living needs.
We can also help with tenancy support and managing your home. See Accommodation and Living.
We support people with many types of disability, including intellectual disability, autism, psychosocial disability, physical disability and acquired brain injury. Tell us about your needs, and we will be honest about whether we are the right fit.

Our prices follow the NDIS Pricing Arrangements and Price Limits, and we set them out in your service agreement before supports begin. Most people pay from their NDIS plan, whether it is NDIA-managed, plan-managed or self-managed. Personal Training funding depends on your plan and goals, so we help you check first.
Yes. Your service agreement explains notice periods and any short-notice cancellation charges, which follow the NDIS Pricing Arrangements. You can end your agreement at any time in line with its terms. If a worker is not the right fit for you, tell us and we will work on a change.
We collect only the information we need, store it securely and share it only with your consent or when the law requires it. You can ask to see or correct your information at any time. Read our Privacy Policy.

We are a registered NDIS provider. This means we follow the NDIS rules for safe and good support.

Support Coordination. We help you understand your NDIS plan and find the right supports.
Psychosocial Recovery Coaching. A coach helps you when your mental health makes everyday life hard.
Daily Living Support. We help with things you do every day, like showering, dressing, cooking and taking your medicine.
Household Tasks. We help with jobs at home, like cleaning, washing clothes and mowing the lawn.
Community Participation. We help you go out and do things you enjoy, like shopping, sport and seeing friends.
Travel and Transport. We help you get to places like appointments, work and study. We can teach you to use buses and trains.
Accommodation and Living. We help you find a place to stay, for a short time or to live long term.
Home Modifications. We help make changes to your home, like rails and ramps, so it is safer and easier to use.
Personal Training. A trainer helps you get fit and strong in a way that suits your body.

Someone else can contact us for you, like a family member or your Support Coordinator.
We talk with you about your goals. Goals are things you want to do or get better at.
We look at your NDIS plan with you. We check what your plan pays for.
We make a service agreement with you. This is a paper that says what support you get and what it costs.
You can have an interpreter. An interpreter is a person who helps you talk in your language.
You can bring a family member or friend to meet us.
You have rights. Rights are rules about how people must treat you.
You have the right to be treated with respect.
You can make your own choices.
You can choose who supports you.
You can say no.
You can change your mind at any time.
You can have an advocate. An advocate is a person who helps you speak up.
You have the right to be safe.
Our workers have an NDIS Worker Screening Check. This check helps make sure they are safe to work with you.
No one should hurt you, yell at you or take your money.

We will listen and try to fix the problem.

Lenity Care Australia has zero tolerance for abuse, neglect, exploitation, discrimination or violence towards any person we support. We follow the NDIS Code of Conduct, the NDIS Practice Standards and the NDIS (Incident Management and Reportable Incidents) Rules.
Our workers must never use physical, verbal, emotional, sexual or financial abuse; neglect a person's needs; take money, gifts or property; have inappropriate relationships with participants; or ignore or cover up concerns. Breaches lead to disciplinary action and reporting to authorities.
